A plain-language summary of the goals, design and what participants do
The trial focuses on advanced gastric cancer, a late‑stage stomach tumor that has spread beyond the original site. Participants will receive an intravenous infusion of Rilvegostomig (code name AZD2936) and may also be given pembrolizumab, a drug that blocks the immune‑system protein PD-1. The study also explores blocking another protein called TIGIT to see if the combined effect improves the body’s ability to fight the cancer. The purpose of the study is to understand how these treatments change the immune response both in the tumor and throughout the body.
Patients will have a short “window‑of‑opportunity” period during which a small tissue sample (biopsy) is taken before the first infusion and another after the initial dose to look at immune biomarkers, which are signals that indicate how the immune system is reacting. After this phase, regular infusions continue, and doctors perform routine scans and blood tests to monitor the disease. Outcomes such as how long the cancer stays stable without growing (progression-free survival), how many patients see their tumors shrink (overall response rate), and the presence of tumor DNA fragments in the blood (ctDNA) are recorded. Additional laboratory analyses may examine genetic markers like HLA, immune‑activating proteins such as IFN-γ, the presence of immune cells marked by CD8, and the level of a protein called PD-L1 using a staining technique known as IHC. Safety is also closely watched through regular health checks and reporting of any side effects.
The trial runs in 8 steps – from screening to follow-up. Each step says what happens and what the team monitors.

is an experimental medicine being tested for advanced gastric cancer. It is given through an IV infusion and works by blocking two proteins, PD‑1 and TIGIT, that normally keep the immune system from attacking cancer cells. By blocking these proteins, the drug aims to boost the body’s own immune response both at the tumor site and throughout the body, helping the immune system recognize and destroy the cancer.
(pembrolizumab) is an approved cancer immunotherapy used as a comparison treatment in the study. It is also given by IV infusion and works by blocking the PD‑1 protein, which allows immune cells to better detect and attack cancer cells. In this trial, it serves as a standard reference to see how the new drug’s effects differ from an existing therapy.
It is given by IV infusion as a solution for infusion, usually 750 mg per dose. Rilvegostomig is an experimental drug still being studied in clinical trials and is not yet approved for regular medical use. It is being tested for treating advanced gastric cancer by blocking two immune checkpoints called PD‑1 and TIGIT, which helps the immune system recognize and attack cancer cells. The drug belongs to the class of immune‑checkpoint inhibitors.
It is administered by IV infusion as a concentrate for solution, typically 200 mg per dose. Pembrolizumab is an approved medication widely used in cancer therapy and is listed in medical literature as a standard treatment. It is used for advanced gastric cancer and other solid tumors by blocking the PD‑1 protein, allowing immune cells to target cancer. It is classified as a monoclonal antibody immune‑checkpoint inhibitor.
